Hybrid system failed - "System Malfunction Visit Your Dealer"

When I decided to test drive both the Hybrid and non Hybrid from fresh off the truck, I was told I had to wait about an hour for the Hybrid to be prepped. I was told service had to go through it and prep the Hybrids before it was clear to drive….. ??‍??‍

As I was waiting to get on the freeway I heard a thump like the vehicle shifted out of gear. Luckily we were in traffic and crawling and the Toyota dealership was 3 exits up when I hit 30 mph my rpm's were at 4000. Waiting to hear today. My platinum only has 1600 miles on it.

I am waiting for my hybrid. I owned my Limited since new in 1997. The car was bulletproof and reliable now this has me worried. I had a Jeep before my 4Runner and it was in the shop every 3 weeks. I should had waited a year with this new engine but I had planned for a long time to buy a new 4Runner. I am getting a Limited hybrid. Did Toyota test these new engines out?

Their not an entirely new engine, just new to the Runner and Taco lineup. I believe they've been run in the Lexus line.

I had the Hybrid System Malfunction code come up on my 2024 Tacoma Trailhunter with less than a thousand miles. Toyota ended up replacing the Transmission, Torque Converter and Hybrid battery. They ended up doing a recall on certain Vin numbers. Hopefully this isn't happening on the Hybrids 4Runner. The good news is the truck was only in the shop for a couple of days and now hitting 10,000 miles without any problems.

2025 TH under 1,000 miles. It’s been at the dealer for 2 weeks. They think it’s the inverter assembly which is throwing the codes.

Here I am waiting for my TRD pro and I’m seeing this. I wonder if it’s just a bad batch of 4Runner hybrids. This is still not okay Toyota.

Going on 6500 miles on my ORP Hybrid…. So far perfect..!!!! Love it..!!!!

Happened to me on my ORP hybrid. 1100 miles. Drove to the dealer and of course no error. They ran diagnostics didnt find any issues.

Well I unfortunately had something very similar happen to me on Saturday at 1523mi, just dropped it off at the dealership this morning. @CyrusJones am curious if your issue was also the Inverter Assembly or if it was something else?

Was running errands around town and all of a sudden while driving the car revved up and threw a bunch of alerts including:
- "Hybrid System Malfunction"
- "Drive-Start Control Malfunction"
- "Advanced Ultrasonic Detecting and Ranging System Malfunction"

Thankfully was less than a mile from home, it was extremely sluggish, I still could drive up to the speed limit but seemed to really hold the RPMs before shifting in order to accelerate. Also noticed all the adaptive cruise control/emergency braking/etc were orange so I guess that is the "Advanced Ultrasonic" system.

This morning, when I turned the ACC on all the same warnings were on the dash, but as if it knew I was taking it in, it started up like normal and I watched all the dash lights shut off one by one.

Drove like normal, hybrid and everything working so who knows if the techs will be able to find anything. It still had the alerts in the car history with date and time so hopefully there is some data they can pull, keeping my fingers crossed and will update when I hear something!

I'm fearful they're going to tell me the same thing, have you had issues since?
