I think it’s probably something simple like the radar is misaligned in the grille. I do think in the video it’s using dynamic cruise control because of the blue lines but the good news is it could be an easy fix and you’re staring at a ton of warning lights. Bad news is you’ve gotta take the thing apart again.
Yeah I hope it’s something simple, when I took it to Toyota and they did the initial scans they told me no codes or warnings were thrown. I need to disconnect the Dswitch and some other things before I bring it in again since they didn’t know how it was wired and didn’t want to touch it. Really annoying issue considering I use cruise daily for my work commute 50+ miles a day.

Yeah tested with both, here’s another video of the issue and both modes:

I notice that the speed limit is 65 but you've set cruise control for 75. is it possible you have Road Sign Assist (RSA) that can work with Dynamic Radar Cruise Control (DRCC) to automatically adjust your cruise speed to match the detected speed limit? I don't know if your video shows normal Road Sign Assist (RSA) behavior or not.

dynamic and regular CC ignores RSA. only thing it will do is throw up a red icon(in the bottom left of the MID display) if you go more than 3-5 mph over the RSA speed limit

OK. Well, I sure am glad mine does not do that. It would bug the heck out of me. Hope we end up hearing what the final resolution is.

Hmm, I do notice the speed sign icon turns red when I'm exceeding it, but my cruise control works normally.

RESOLVED:

Discovered the issue! When I got the 4Runner there were light add ons, the dealership who installed these aftermarket lights tapped the ENG-IGR #2 Fuse in the driver side fuze box. Thought this was a random fuze they tapped, ends up it is a mandatory fuze that is used. Oddly I did not get any codes/issues that came up while not having this fuze installed. Ends up though after putting the fuze back into ENG-IGR #2, the cruise control speed maintained. Apparently this fuze maintains the cruise control speed for future reference.
