Okay, so you ran cable to the radio along the driver's side. Did you go under the carpet? Under the plastic at the door? Past the center console?

I'd recommend going the route of using a cowl mount.
1) The antenna won't be as high so that you can get the truck in to lower places.
2) If you do hit the antenna on a branch of a tree, or a parking garage you'll immediately be able to see what's going on.
This is my second 4Runner where I've used a cowl mount and I've had good performance both times.
Oh, the last reason I like that mount is that if I'm going in to a sketchy area I can easily reach the antenna, unscrew it and cap the connector.
