I'm leaning towards #3... Probably a setting you might be missing. I have pixel pro 10 and TRD ORP and it connects wirelessly (Bluetooth) to Android Auto 95% of the time when I start the car. Occasionally I have to force it with the toggle in the Toyota connection settings in the infotainment.
I had an aftermarket subwoofer (and dedicated amp) installed on my ORP. It taps into the software signal from the OEM sub. Fwiw, when I fade all the way to the rear my aftermarket subwoofer signal is still the same and sounds no different. So this doesn't make me think it's a software issue with...
Probably not related but, the new Lexus GX 550 is experiencing a rear audio fade widespread issue. There is a huge online thread discussing the potential cause, which appears to be related to the amplifier and/or the rear fade software.

I am at 4700, mi and it still does it to me every single cold start. And I don't baby it. It is what it is and I'm not going to worry about it anymore.
I think you have them installed correctly. I have had them on since day one (dealership installed). I definitely notice the wind noise at higher speeds. Been debating whether it's worth keeping them (just for the looks)...or to take them off bc they're so noisy.
Is it normal to hear a pretty significant clunk when going from 4h to 2h? I've been in 4h five times now and each time I've returned to 2h I've gotten the clunk. I've tried returning to 2h in neutral as well as in drive and both still gives me the clunk.

For me, it's definitely not the seatbelt. It is behind the door panel near the elbow rest. Rattle occurs at certain bass frequencies in same location on both front doors. There is another thread discussing it and someone posted pics with the passenger side door panel off. I've attached one...
